Affiliation Agreements

The Attorney General’s Office has approved standard language for the Health Sciences Affiliation Agreements. The approved agreement templates for each Health Science School is linked at the bottom of this page. They are current as of June 2025. As Health Sciences Services continues to refine procedures in support of the Health Science Schools, we request the following when presenting Affiliation Agreements for review and signature:

Agreements with changes to the standard template and/or training site generated templates:

  • Please submit requests for the Attorney General Office (AGO) Affiliation Agreement Review Request Form as part of the Review Process section of the UW AGO Affiliation Agreement. Because the AGO is not a party in affiliation agreements, the AAG’s signature is not a legal requirement. Therefore, the UW Division of the Attorney General’s Office no longer maintains the practice of signing affiliation agreements. Approval as to form is conveyed by email.
  • AAG overseeing AAs is Erica Graser. Please review the AGO’s Affiliation Agreement FAQs section for answers to commonly asked questions. Insurance-related inquiries should be directed to Risk Services. Program-specific/operational-level questions should be first be addressed within the Program’s department/school and HSS. For other questions regarding AA review, please contact Erica Graser (AAG) and Brendon McCarroll (Paralegal) at: uwagoaa@atg.wa.gov
  • To complete review, please allow up to 4-6 weeks for review of each agreement
  • Due to the familiarity with UW generated AAs, those that use a UW template are usually processed quicker than contacts generated by the affiliate. Although it is not a legal requirement, AGO-approved UW agreements should be considered a prerequisite when the UW is the training site.
  • After routing for internal signatures, please route to Lisa Plancich, at Health Sciences Strategic Planning & Development, for signature from HSS Director Jason Lindahl
  • Expect three (3) business days for HSS review, signature, and return

Agreements with no changes to the language in the AGO approved standard template and/or the AAG has already signed the document:

  • After routing for signatures related to the specific school’s agreement, please route to Lisa Plancich, at Health Sciences Strategic Planning & Development. Jason Lindahl, HSS Director, will sign after all other signatures/approvals are placed on the agreement.
  • Expect three (3) business days for HSS review, signature, and return
  • Include explanatory email or attach a cover page

Standardized Patients

The University of Washington Standardized Patient Program recruits, trains, and supervises a pool of experienced role-players, or Standardized Patients (SPs), and assists with all related curriculum development and course and examination design.

Standardized Patients supplement, rather than replace, real patients. They are specifically trained to present the medical history, simulate the physical symptoms, and portray the emotions of the original patient upon whom their case was based. SPs are also trained to give students immediate, one-to-one rating and feedback on their performance.

The UW SP Program is always available to assist with planning and presenting instructional programs, assessments, and research studies involving SPs.

Currently, we administer and develop SP projects and exams at the University of Washington for many courses and programs, including:

  • Introduction to Clinical Medicine, Years I and II
  • School of Medicine Objective-Structured Clinical Examination Year II and Year IV
  • School of Medicine Chronic Care clerkship
  • MEDEX Northwest Physician Assistant program
  • Nursing graduate
  • School of Pharmacy
  • School of Social Work

We are also involved in a number of research projects and continuing education programs in the larger education and health care community.

Types of Programs

  • H&P Teaching patients
  • Group encounters
  • Mannequin/model-based Tutorials
  • End of Life, Error Disclosure and Difficult Conversations
  • Unannounced SPs
  • Motivational Interviewing
  • Team/conflict skills training

Health Sciences Services

In addition to working with all six Health Science Schools (Dentistry, Medicine, Nursing, Pharmacy, Public Health and Social Work), Health Sciences Services (HSS) manages the South Campus buildings, classroom scheduling and services, Interprofessional Education (IPE), Scientific Instruments, and UW Photography.

Health Sciences Services oversees the UW Standardized Patient Program. This program recruits, trains, and supervises a pool of experienced role-players, or “Standardized Patients” (SPs).

All Affiliation Agreements for Health Science Schools route through HSS. Please review the Affiliation Agreements for reference, Health Sciences school templates, and Attorney General Office direction.
